Every change names the person who made it, and when

The name on a change is the account that made it, not the employee whose entry it was, so a correction is never anonymous.

Where a change came from

The record shows whether a manager made the change or an employee asked for it.

A manager can correct an entry outright. An employee can ask instead, with a reason attached, and nothing moves until someone decides. Both paths land in the same record, and a request you turn down stays there with the reason you gave.

Where do I approve or turn down a correction?

In Edit Requests, where the request arrives with the employee’s reason attached. Your decision and any note you add go onto the record, so the audit report afterwards shows what was asked for, what you decided, and who decided it.

Who can open the audit trail?

Administrators. Employees can see and request corrections to their own time, but the report that lists every change across the workspace is Administrator-only.

How far back does the record go?

The report covers whatever period you pick. A change is kept with the time entry it belongs to, so an older period still shows the changes made to it.

Does an audit trail satisfy my recordkeeping obligations?

Not on its own. Clockspot records the changes made to employee time here and shows you who made them. What you are required to keep, and for how long, varies by state and industry, so treat this as a record you can read rather than as advice about your obligations.
