Does an employee have to submit before a manager can approve?

No. The Not submitted view shows employees without a submission for the selected period, and a manager can approve the recorded time directly as long as every entry in that period is clocked out. An Administrator limited to assigned employees sees and approves only those employees.

Can a timesheet be approved while a shift is still running?

No. The employee card identifies open entries, and Clockspot requires those entries to be clocked out before the timesheet can be approved.

Is there a record of who changed a time card?

Yes. The Timesheet Audit report records each edit with the values before and after the change, who made it, and whether it came from an approved edit request.

How do I know a report only contains approved hours?

Hours reports carry a Timesheet approval banner that says how many employees have approval for the period and lists the ones who do not, with a link straight to the approvals queue.
